I'm not entirely sure if this is just me, but...

When you're calibrating your board, if you're right handed, make sure that you click ever so slightly to the right of the crosshair and if you're left-handed, click ever so slightly to the left of it. This'll ensure that the mouse pointer is directly under your pen.

(I think this might be because I hold the pen at a 45 degree angle to the board rather than straight-on as I find it easier to use it that way - plus my hand doesn't get in the way).

I'll call yu crazy if you like Dave :D :D But it's a good point otherwise the cursor is usually a couple of millimetres out.
